Insurance and Fees

I am a preferred provider with Regence and Premera/Blue Cross Blue Shield and am an considered an out of network provider for most other insurance plans. I am not a Medicare provider and am unable to see Medicare patients.

The extent to which your visits will be covered by insurance depends on your individual plan. Therefore, prior to your first appointment I recommend that you check with your insurance company regarding coverage for PT treatment or my billing service can check your benefits for you. If you would prefer this, please contact my office and we will be happy to do this for you. The fees for your visit will be billed to your insurance company and any fees you are responsible for will be billed to you on a monthly basis.

My fees are time and service based, as follows:

• Initial Evaluation, 60-75 minutes, ($140)
• Follow up visits, generally 60 minutes, ($120)
• Occasionally follow up visits are shorter,
• 30 minutes ($60) or 45 minutes ($90)
• $40 charge for cancellations with less than 24 hours notice or for ‘no shows.’
